Self Referral

Most referrals to other specialists will require you seeing or speaking with a GP. However, there are a number of local services, listed below, that you are able to make self referrals to.

Audiology / Hearing Loss

Access the NHS funded community Audiology providers available across south east London by visiting the Patient Self Assessment website. You will first need to check your suitability for the service by visiting and completing the patient self-assessment form.

Lewisham Talking Therapies

If you’re feeling low, anxious or stressed, you’ve come to the right place for help. Lewisham Talking Therapies is a free and confidential NHS service that is part of the Improving Access to Psychological Therapies (IAPT) program.

We provide a wide range of foot health services at University Hospital Lewisham and in the community. This includes assessment, diagnosis, treatment, education and advice on disease and deformities related to the foot and lower limb.

Youth Wellbeing Clinic

Woolstone Medical Centre working with Aplos Health PCN are proud to announce the relaunch of our Youth Wellbeing Clinic from Thursday 27th January 2022. This service is available to all patients aged 11 – 19-year-olds, who are registered at Sydenham Green Group Practice, The Vale Medical Centre, Woolstone Medical Centre or Wells Park Practice.
