Register with us as a New Patient

Accountable / Named GP for All Patients

All patients registered at Woolstone Medical Centre have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

How to Register

Please complete our online registration form (see below), alternatively you can attend in person you should ideally bring proof of address (Utility Bill etc) and your NHS (National Health Service) number with you when you come to register. You will then be giving a registration form and Patient Questionnaire to complete. After this is returned to the practice you will be given an appointment to see the Health Care Assistant for a New Patient Check, and this would complete the registration process.

If attending in person, we would kindly request that applications are submitted between 12pm and 5pm to enable the registration process to be completed that day. Patients may need to return another day to complete the registration process for applications submitted after this time.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that we will only register children (under 16s) if their parent(s) or legal guardian(s) are also registered or registering with the Practice.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

Not in our catchment area?

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

As a patient, you have the right to express preference as to which GP you wish to see. However, if you require an appointment and your preferred GP is not available, you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.

Register online

The online registration form collects your details, a brief outline of your medical history and a record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please download and complete the forms below, and bring in to the surgery. We must have the GMS1 form and the Registration Questionnaire completed to register a new patient. For children under the age of 6, please bring the child’s red immunisation record when registering.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
