Our aim is to provide high quality health care in a safe and accessible fashion where patients are treated with care and consideration. We encourage patients to take responsibility for their own health and strive to enable the personal and professional development of the staff.
Our Ethos and Values
The Woolstone Medical Centre is a General Practice Partnership open to all patients living within our Practice boundary and the surrounding areas. We work in partnership with our patients and our Patient Forum to provide medical care for our patients.
We are a Personal Medical Services (PMS) Practice offering Primary care services for the diagnosis and prevention of disease. We help patients to manage their health and prevent illness. Our GPs assess, diagnose, treat and manage illness. They carry out screening for some diseases and promote general health and wellbeing. Our GPs and staff act as a patient’s advocate, supporting and representing a patient’s best interests to ensure they receive the best and most appropriate health and/or social care. Our GPs also provide the link to further health services and work closely with other healthcare colleagues. They may arrange hospital admissions and referrals to other services and specialists and they link with secondary and community services about patient care, taking advice and sharing information where needed. They also collect and record important information from other healthcare professionals involved in the treatment of our patients.
Our GPs are also involved in the education and training of doctors, practice staff and other healthcare professionals.
The Woolstone Medical Centres (WMC) aim is to provide the best possible quality service for our patients within a confidential and safe environment through effective collaboration and teamwork and to show our patients courtesy and respect at all times irrespective of ethnic origin, religious belief, personal attributes or the nature of the health problem and to involve them in decisions regarding their treatment.
Our objectives are:
To promote good health and well-being to our patients through education and information, in a collaborative fashion, utilising electronic processes wherever possible to make care and information more accessible.
To involve and collaborate in multidisciplinary team work including nursing and other allied healthcare professionals, social care and the voluntary sector in the care of our patients.
To encourage our patients to get involved in the practice through our PPG, an annual survey and encouragement to comment on the care they receive.
To ensure that all members of the team have the right skills and training to carry out their duties competently and they have opportunities to discuss and learn from problems or issues that arise at any time.
To provide safe, effective health primary care services in a responsive way, meeting the needs of our patients.
To support continuity of care – wherever possible through personal continuity, but also through medical record continuity enabled by high quality timely medical records; and following guidelines based on best evidence, national, and local policy;
To be an active and responsible member in our local health community ensuring our practice and services to our patients are commissioned and provided in a way most likely to meet their needs.
To ensure the practice is compliant with relevant legislation and policy relevant to maintaining trust and confidentiality, as well as ensuring we practice high quality medicine.
To provide a learning environment where we train student and health professionals and are involved in teaching and research. This learning and continual improvement ethos runs through everything we do, we look to continuously make incremental improvements and learn lessons from delivering primary health care.
We have been a training practice since 2004 and have had GP trainees and F2 doctors in the practice.
